thanks for sharing your thought, 
in my previous company we were following the 3rd point. as you already mentioned, when team already went through few sprints they know what their product needs more attention. 
so team can decide their default priorities during sprint planning meeting. we obviously discuss with PO over phone to confirm. 

when we found this kinda problem, we also sometimes took sprint for resolving our technical dept (http://martinfowler.com/bliki/TechnicalDebt.html)
